TIMELINE

Timeline is a type of report that identifies only the key events related to the allegations over a defined period of time in a graph format. A Medical Chronology might include notations about all medical care that the plaintiff received whereas a timeline focuses on a few facts over a specified time period. Timelines are often used as trial exhibits to show jurors key elements of the case.

MEDICAL SUMMARY

Our Medical Summaries not only provide a narrative report of the patient's course of medical events but also an analysis of the medical/legal issues in the patient’s medical history. The purpose of the analysis is not to dazzle the attorney with in-depth knowledge of the medical issues. A Deposition Summary provides a brief, concise statement of the main points of the deposition. We provide both page-line summaries and narrative summaries. The Page-Line Summaries, identify important points in the deposition by page and line. This summary is useful for attorneys who want to skim the summary or compare one summary to another.Narrative Deposition Summaries are similar to narrative medical summaries. The testimony is described in a narrative format along with the description of salient part of the deponents.

Treater Lists

A Treater List is a concise and comprehensive accounting of individuals who have interacted with the patient. The chances are high that treater names supplied during discovery will be incomplete. Without a centralized documentation system, attorneys have to rely on the patient and defendants to supply them with treater names. WOUND SIZING

Measurement of wounds are critical in managing diabetic and decubitus ulcer patients and recording them precisely is even more critical. Wounds are tracked by their size in centimetre. Our report describes wounds in both centimeters and inches. We go an extra step in comparing the wound with common objects that everyone is likely to understand. The sample has a list of common sizes in centimetres and objects corresponding to that size. Due to rounding, some sizes appear more than once.

Failure to attend to the patient or nursing home resident over a specific period of time are common not only in medical malpractice and nursing home neglect cases but also in cases alleging inadequate staffing. Touch Charts are graphic representations of the number and type of health care providers that rendered care to a patient or resident over a defined period of time. The time frame for a Touch Chart can be developed for periods ranging from a few minutes to months. Because Touch Charts are time specific, the information is often presented in a clock, calendar or day planner format. A Touch Chart can be used to refute claims that the patient was not coded and was only treated by limited staffs in the last few minutes of his/her life.
